Thank you for your participation in the 2013 Annual DD Employment & Day Activity Outcomes Survey.
 

Why the survey is important?

The state of RI has identified employment as a priority service outcome for people with DD/ID.  There have been changes to service delivery expectations and how services are funded over the past few years. Participating in the 2013 is important so that we can build on the outcome data from 2011 and 2012. 

The survey results reported in the Statewide Data summary informs us of overall trends and helps guide our collective efforts to improve employment of the individuals we serve.  Additionally, the individual agency reports (day service, residential, and agency total), provided by the Sherlock Center, support your agency’s quality improvement efforts.


Data Entry Directions:
1. It is recommended that you review the directions and gather the required data prior to starting the on-line survey. A copy of the survey directions are provided for your convenience.    Download Survey Directions

2. Complete 1 Survey Per Individual
NOTE:
If you need to complete a survey over multiple sessions, you can click the "SAVE AND CONTINUE SURVEY LATER" button at the top of the page. You will need to provide an email address in order to access the survey at a later time. Click to a new page to save all data entered up to that point.
